Ratings are made on comparisons between shows of similar genres and category. I dont get too technical with ratings most of the time and do it by feel thinking about what Ive seen before thats similar in score and go from there. However at both ends of the scale I do tend to be more critical of shows and strict with my reasoning. I write in reviews and notes when I feel inspired to do so especially when a show is complex and I want to remember my thoughts on it.
